Retatrutide is a synthetic peptide developed as a triple receptor agonist — designed to act on three distinct hormone receptors at once: the GLP-1 receptor, the GIP receptor, and the glucagon receptor. These three pathways are each involved in the regulation of blood sugar, appetite, and energy metabolism, and engaging all three simultaneously is the feature that distinguishes Retatrutide from earlier compounds that act on only one or two of them.

Much of the scientific attention given to Retatrutide stems from this combined mode of action, and its Phase 3 clinical program has produced notable findings. In the Phase 3 TRIUMPH-1 obesity trial, high-dose (12 mg) treatment produced a mean weight loss of 28.3% at 80 weeks, with 45.3% of participants achieving at least 30% weight loss and 65.3% reaching a BMI below 30 kg/m². This was accompanied by cardiometabolic improvements, including a 24.1 cm reduction in waist circumference. In its first Phase 3 type 2 diabetes trial (TRANSCEND-T2D-1), participants achieved average A1C reductions of up to 2.0%, alongside weight loss, compared with placebo.
